"15 items or fewer" is an instance of hypercorrection. "or less" has been the preferred usage following count nouns for centuries. (Try Google Books search for numerous 19th century instances. "or fewer" is rarely used)
I understand that correct grammar would be "15 items or fewer", but "15 items or less" has become common usage and actually sounds more natural. Besides, if you were to point this out to your local grocery store, they would change the sign to "No more than 15 items", which also happens to be correct.
theres a sign that I drive by sometimes, it says Slow Children Playing... every time I am afraid some slow child will walk in front of my car or something. It should read Slow Down: Children are Playing

"less" applies to non-countable nouns, "fewer" applies to countable nouns.
It would be something like 150 lbs. or less, but it's 15 items or fewer
