Humid air from outside the rink gets in when the doors are open and condenses into fog above the cold ice. Kind of like in the freezer aisle at the grocery store, you open the door and all of a sudden it fogs up.

Usually it gets foggy when the dehumidifier's shut off or when there's a drastic temp change outside that covers the rafters in condensation. It used to happen at a couple of the rinks where I played in St Louis. It gets really humid in the Midwest and the weather can be pretty schizophrenic from day to day.
