I have a crown with an infection at the tip of the nerve, i think i have a root canal there..its my front tooth...I am doing apicoectomy/surgery to treat it and came across this video...i was recommended an implant by the first dentist and apicoectomy by the second...what is the best solution?? i have to do something about the infection, and also the the tooth is getting darker by the day..WHAT can i do??

I have never seen one scientific published study which verifies the claim that Calcium Oxide will sterilise a tooth. the theory is good but the practical is not. Sterility is only one of the issues - It is impossible to sterilise, to remove all dead gangrenous material and also to seal the tooth. None of the aims of rct are achievable. Presumably the answer is that if the original dressing needed a mixture of chlorhixidine and calcium hydroxide (because of e faecalis) the subsequent dressing will be unable to control e faecalis re-invading the micro tubles so this approach is perhaps safer as the final sealant is less toxic but not a viable solution because of e faecalis reinfection. A lot of dentists are now proposing this as a safe root canal alternative.
The theory being that the two initial 4 weeks treatments could 'sterilise' all the dental tubules (tests having shown this causes the whole outside of the tooth to acquire a high alkalinity thus, supposedly the high ph has killed the bacteria) and the calcium oxide would, having set, turn back into calcium hydroxide in the presence of re-invading bacteria and re-steralise the tooth before re-setting. Is this a viable alternative?
Namely, using a calcium hydroxide and chlorhixidine mixture (as the former doesn't work on e.faecalis but the latter does) and leave this in the tooth for 4 weeks followed by a re- treatment for another 4 weeks and then fill the canal with calcium oxide (Biocalex) a far safer sealant than traditional gutta percha.
(4) I have a teenage son who has a badly cracked tooth and another abscessed tooth. Naturally enough, at his age, he does not want to extract and get a false tooth (nesbit) or harm adjacent teeth with a bridge or use orthodontics to move other teeth into the gap in the hope that his wisdom tooth erupts and can be moved into the subsequent space (because of nickel, head, neck issues). He wants to know if the 'so called' biological root canal option is a viable solution.
Failure to recognise that just because a root canal treated tooth no longer hurts or radiographically does not appear abscessed does not mean the tooth is not a source of systemic infection is an exercise is self delusion.
I would like to ask a question of the author about the so called alternative 'biological' root canal treatment.
(2) I have had a few teeth extracted in preference to doing root canals due to the cytotoxicity of both the root canal filling material used and the inability to sterilise a tooth due to the bacteria which inevitably remain in the kilometres/miles of micro tubles in the tooth after endodontic (root canal) treatment.

I had a root canal retreated by a top class endodonist. It's niggled me ever since and I'm sure it's a drag on my health. Having read up on the problems with the treatment, while some stuff out there seems sensationalist, I have to say it chimes with my own experience. The statements in this video are completely disproportional to the facts of root canal therapy. It is like still insisting the earth is flat. Root canal therapy is the only way to keep your tooth and done PROPERLY is over 95% successful, over 97% of people keep their teeth, and most of the rest fail due to anatomical complications or poor follow-up, but upon retreatment even those 3-5% have close to 90% chance to succees.

Yes I am a dentist for 30 years. did thousands of rcts - make no appology for the movie or its contents and if you care to check it out you will see that it is fully referenced. In fact most of what I am saying is even being acknowledged by the Aust Dental Assoc as being correct. Unfortunately we are taught rubbish at uni and the basic premis that we can sterilise a tooth, remove all dead tissue from the tooth or even completely seal a root canal is not supported by any of the science.

to tristan801 - please go to the website advertised at the begining of hte trailer and check out the references and support documentation, and then tell me where I am incorrect. Otherwise you may enlighten all of us and tell how you test to see if a tooth is sterile before filling it

Yes I am a dentist for 30 years. did thousands of rcts - make no appology for the movie or its contents and if you care to check it out you will see that it is fully referenced. In fact most of what I am saying is even being acknowledged by the Aust Dental Assoc as being correct. Unfortunately we are taught rubbish at uni and the basic premis that we can sterilise a tooth, remove all dead tissue from the tooth or even completely seal a root canal is not supported by any of the science.

Thank you for your comments. I hope that you are really a dentist and really serious. There is a great deal more. what you have been looking at is only the trailer of a full referenced one hour doco. It is important to understand the implicaitons of the misinformation we are given as dental students. The paradigm needs to shift

One of the most impressive docos I have ever seen... whoever did the animation - incredible stuff. Great to see a doco that is so well referenced. I was lucky to have my one root therapy taken out some years ago and my arthritis completely disapeared within one month
- It started a few months after I had the root therapy done several years earlier. Thanks for making this information available.

saving a tooth is not a good enough reason for killing the person who is stuck with the root treted tooth. How about you give us some real stats about how you measure success.

would you care to tell the world how you know that you have sterilised a tooth, removed all dead tissue from the tooth and managed to completely fill the tooth. What follow up do you offer to check the health status of your patients? WHich criteria do you use to measure your 95% success rate. WHich criteria do you use to measure your 5% failure rate.
