You would need to either crit, or have both spies already be almost dead from someone else's damage. Otherwise it's not possible. The SMG doesn't do enough damage.

This sort of thing isn't that rare. It'll happen to some spy just through sheer luck alone like at least once every map rotation.

A fully charged sniper shot anywhere to the body will kill a spy at full health (cloaked or not, of course), and it's a common thing for snipers to watch narrow bottlenecks with long sight-lines (like in this video, actually). This makes it a lot more likely to just luckily get one of these types of kills.
