You have to remove the metal housing on top which allows you to lift the ring used to adjust the falme off of it's gear so that you can continue to loosen then gear allowing more fuel to be released than what was meant for.
You do know when you hold down the lighter for too long, when the flame is adjusted the flame catches below the lighter top, igniting the lighter fluid inside the lighter causing it to explode? I did it. xD

BICs generally aren't adjustable. You need to find adjustable lighters. They're a bit cheaper too.
