Management wants to release the product quickly. Engineering wants to get everything perfect before release.

The proper balance has to be found between the two, and that isn't always in the middle.

With Challenger and her crew at stake, that balance was closer to the Engineering side, but tragically wasn't seen that way...

The cause of the explosion was a failure of the afl joint seal in the right SRB due to cold weather. Also, Challenger did not explode itself, but various structural failures caused the orbiter to break apart. It fell in the Atlantic Ocean, it impacted, and killed the 7 crew. :(

@sideslide23 It's about five miles between the crew quarters and the launch pad, so they need to go by car. The "astrovan," as it's called, is large enough for the crew and their equipment, as well as anyone else who need to join them, and it (or a similar vehicle) has been used since the Apollo era. There's no particular reason for its color or make and model, but apparently, the astronauts like that it's so full of history (having helped launch every shuttle since 1984).
