I'm afraid I don't have plans, but that's probably best. Details vary depending on the situation. It's really just a matter of making two pieces of 1/4" ply, extending them to fill the width and angle of the space, then clamping them together as one. If you try it a few times you'll see where you need to cut and modify you wood.
Another idea was taught to me by a boat builder. Use 1/4" (thick)strips -about an inch or two wide - place them all around the outside edge and "hot glue" them together. The glue dries almost instantly and you have a perfect template.
