It was a ground out, so when throwing to 2nd to tag out the 1st-to2nd runner, it's just 1 out, to 1st again to tag the home-to-1st runner, just 2 out... where's the third out?

wasn't a ground out. the 1st baseman caught the ball.

runner on 2nd went to 3rd so he threw to 2nd for the out and the runner on 1st was off the bag so he threw it back to 1st for the 3rd out.
