In underdeveloped countries families often have 10 kids, not b/c they love kids, but b/c 8 will probably die young from disease. Give those families life-saving vaccines, and they usually have fewer kids.

I suspect that many couples in poorer countries also purposely have many children because the parents are relying on at least one of their children to do well so that they will be supported in their old age. Often, their other children, due to lack of opportunities and resources, are forced into the same awful environment as their parents.

Fewer children survive to adulthood and many more are born to support parents in old age. Better vaccines lower the risk of death, especially at a young age.
Better health care should mean that parents will not need to rely on their children to physically care for them in old age.
