No there is no ceremony. for one to become a muslim He or she has to say "Lailaha ila-allah Muhammed roosul Allah" which means there is no object , god ,diety or anything that one conciders a god to be worshipped except one true God the creator of Heavens and earth and Muhammed (Pbuh) is his massenger.

Children are already muslim until they reach the age of understanding, after which they are more concious of their actions and deviate onto other faiths such as their parents(if non muslim). Every human is born in the state of islam, (submission to the true god).  So your children are already muslim untill you deviate them from true path. After that they will need to revert (not convert) to islam when they reach their age of understanding and the concept of one god as mentioned.

It has something to do with hygene and serenity. When you are going to pray, you need to wash up too, your arms your feet your head. There are rules on how to do that too, and prays that you need to say in between. So when you want to pray, which means thats 5 times a day for a muslim, your body and your soul must be pure. I guess it has something to do with this.
