1. The video is meant for broadcasting. on 5.30 a switch is set on ' hand'. In practice it is then 'on'. In reality it was put in the 'off' postion, which means it can be turned on manually by switching it on by walking to the MCC and turning the knob left to the 'on' position. Field switches these days only steer the MCC thus are not 'hot' switches. If 'hot' switches in the design was the case.
I personally hesitate providing any override at all to safety instrumented systems. This includes manual interference to critical systems. Something learned from experience. All safety systems should live away from MCC if possible.
2. A person had to walk to the device and locally switch it on. This takes a lot of time (for emergency occations) and it is not even sure if the device can be reached (in case of fire). It's all before my time ( I was a child when it happened) and I don't know the exact design of the installation but these are horrible scenarios (I was in Bhopal last yr and did some investigation. that was horrible too).
I am afraid that problem is not only 'greed' but more than that. Process safety requires knowledge sharing and competency. As for local action, that's why we have Rosov! and Bhopal was a string of human errors knitted together for a disaster.
There are many problems. You are right that it is not only greed of shareholders. Lack of competence and knowledgesharing is also a problem but these can (in my opinion) be fed back to economic factors which are (in my opinion again) the responsibilities of shareholders. True, there are a lot of operations technicians that overrule the shut down system. By putting systems on hand by the way, there is still a shut down sytem behind it.
Unfortunately all the holes lined up and people died. Their deaths while senseless were not in vain as a result of the inquiry better procedures were developed and more in depth Risk Analysis are being done
awhile ago i saw the seconds from didaster veryoin of this didaster and it stated the explosion came from sector A where the large storage tanks are but idk if thats tru or not
Thank you for posting Part 1 &2. My Dad was killed on the Piper Alpha when I was 2yrs old. Watching the videos has helped explain the question I ask everyday "WHY?"

Why in the bleeding hell wasn't the switch for the A pump locked out/tagged out? A 1 cent tag would have kept any of this from happening!
