Hey, Ray. No, the Sideshow figures hold together just fin usually, but since this one has a removable arm, this process was necessary. True, they should have put more attention into how this arm would stay on, but this is an easy and trusty fix. I have almost forgotten that mine used to fall out all the time.
The peg itself stays in place on the arm. The problem is that there's nothing to hold the forearms on to the bicep. It means that if you don't "shroom it out" the forearm will fall off when you touch the doll, or sometimes for no reason at all.

Does the arm peg slide off even if you don't twist and turn the swivel?

Just like we did, shroom them both. Just be sure not to hold the peg to the heat for too long, or you'll create a bigger "lip" then you will need.
