The card was in the menu, you can see it in the way he's holding the menu. The people who gave him the menu knew what the card was because they saw it.
i think someone is a shill working with him, most likely whoever hands him the menu. that person has seen the card the man chooses and grabs a duplicate from a deck they've been given. that person slides it in the menu then david does some really smooth slight of hand when he takes the plastic off the menu, and then slides it under the menu prior to stabbing it. blaine is an excellent sleight of hand artist and all around magician, and he makes it all look really faking smooth.
I agree with christo930. It is obvious to any one who does magic that the card was in the menu. But over all a really cool trick

That's why he took so much time making sure everyone saw the card, it gave his assistant the time to pull the right card out of the other deck.
