Its pretty much about a broken relationship. The casino brawl is probably a scene of some of the history, and the songs metaphor. Refering to her as the queen of hearts and the gamble he took on it all.... it seems as though he's far more into her than she is him; and when things don't go right and they fall out he waits for her return. All the lyrics refer to the symbolism of any fight... the time inbetween making up seems like eternity "so much time, so little left to do"...and he is confused
