Madagascar will survive just about any illness and no one would bother attacking Australia. Australia > Madagascar because it's partially civilized, speaks english (which if you're a native english speaker can be important) and has a varied geography, but either would do.

This kind of game; No I wouldn't defend it on a simulator of a disease.

A lot tweaking and population spread map, tourism, trade, and public transportation would have to collaborated from all countries/cities/states/etc, health care, people's average health, people's immune system over a certian area would have to be taken into account for a somewhat accurate simulator to be developed
