you put the motors in the wrong order.

Please tell me u didn't use a c6-5 for a booster... Because if u did, then the charge would have delayed 5 counts and ignited the next stage when the rocket was pointed down.

@FanandCheese dude that rocket doesn't even take a C6-5. it takes 24 mm motors . D's and E's

appears that the booster must have had a delay before the charge went off to light the second stage. What motor did you use for the first stage?
