well this is what I think will happen later in the manga, once pain enters konoha, tsunade will confront and try to fight pain in order to keep him from getting to naruto and also to avenge jiraya, but she will die in battle instead! I think its pretty obvious as this is how tsunade will die. Tsunade will then use up all of her energy with some special justu that she thinks will kill pain, but it will all be in vain and we all will finally get to see tsunade as an old women, then she will die!
