I would suggest not to eat the intestines and enjoy the rest of the fish chilled together with "tunnbröd" (a soft, thin kind of bread), chopped red onion, dill, boiled potato and perhaps some gräddfil (Swedish sourcream). Simply make a wrap of these ingredients and don't forget about the beer and "snaps"!
I'm convinced most people could eat surströmming served in this way, especially if they never were told it's rotten fish.
Fermentation is basically the breaking down of a substance, which is what rotting essentially is. Though the fish isn't rotten, it's definitely rotting.
