Oh, and that thing with Feint Death and Overkill used to happen to me a lot, too. The trick to it is that you'll use Overkill if you're looking through your sights by holding the left trigger, and you'll Feint Death if you aren't.

It's 360. You can tell from the feint death/step jump/overkill prompt, it shows the A button.
