For those that want to build your own...no I'm not answering questions beyond this post nor will I build one for you. I just used Google.
Looks like it could be based off the UAA180 which will drive 12 LED's.
Acrylic sheets can be purchased on-line. Look for "Apple Pie Memories Acrylic Stamp Block-6" x 6" x .5" Thick" (Amazon $5.58 each)
Use a nice frame to hold the pieces together, space them how you want. Search Google for the UAA180 and you'll find the schematic.
Don't ask me questions.
