Lana does die and her death is shown in the car accident which happened in Season 5 Episode 12. Though Clark went back and time and didn't tell her his secret or propose to her and saved her life and prevented her form dieing. Though he couldn't change fate and someone close to him had to die in Lana's plce.

His father died and that's his funeral shown, he ended up having his father die and Lana left him because she couldn't take more secrets, he lost both of them in the end.
