If I ever get top surgery (and I will probably will lol) I would pick the keyhole thing. I have very small boobs. The last time I wore a bra that wasn't a sports bra (long time ago) I had a 32 A. I've heard that once you take horomones and work out your chest will get a little bit smaller too. :]

this guy really calms me. im so fearful of top surgery (afraid ill die or something) but i feel really comfortable watching this video. just something about him. he also obviously knows his stuff.
I suppose it's a matter of opinion. I've seen Johnson's work, which is pretty good, but I also know it can be a huge pain to get a letter from her afterward (hug drawback in my opinion).
I have some friends that have been able to get decent letters from her in a timely manner. but it is true, it's all a matter of opinion. Beauty is in the eye of the beholder.
You might consider sending your subtitling text to the the folks that posted/hosted this video and encourage them to add it to the original video. They may have the ability to do this without re-uploading a modified version of it.
