It seems to me like the main character is getting captured by his Doppelgänger who proceeds to torture him while taking over his life. Notice at 0:15 it's the guy with the beard, but at 3:49 it's the Doppelgänger without the beard.

I think the 'screaming' or 'crying out' would be from the pain of torture as well as the hysteria from going crazy.
