In fracture NOF, quadriceps bulk would be displaced so you'd still get a difference. You'd also need to look out for external rotation. And there's no reason why there'd be significant difference in quadriceps bulk unless some other pathology was going on, which is a notable finding anyway.
Remember in Trendelenburg test ..."Sound side sags".... That means sound side dips below( the side that patient is 'NOT' standing on) .. This indicates that muscles are not working on the side patient 'IS' standing on.
Good tests, however, measurements with tape are unreliable; normal asymmetries of thigh girth will alter measurements. Need radiologic exam for this, I suppose if the difference is over an inch, it's worth checking any way. I'm sure you know this
