Not that much I think. The only true difference is that she's older. The rest is only clothing. That and attitude, I feel that post-PC98 Alice looks a lot more confident with her army of dolls and huge boots.

@BladeTheFirst Thats because in MS, Extra, as you could see, she was using the grimoire. As she was a human it ate up her life, making the windows Alice. Any normal human would have died in hours. And thats why as she used the grimoire, her attacks were full power. The reason of the minimum effect of Alice using the grimoire may have something to do with her being with Shinki, for that time in the Touhou series...

@rockstar1000avi As awesome as that sounds, that is only fan theory. In canon, there is much question whether the first 5 PC-98 games are even connected with Gensokyo's canonical storyline or not.

@Shintear Well, we have Reimu stating to Yuuka that "I've defeated you a long time ago" in POFV which echoes the events of Lotus and Land Story so I think that it is possible that they are connected in some way.
