I use a mouseover focus macro to focus the shambling horror, yes. It is easy to see enrages then because it has a cast bar (focus cast bar enabled of course).

It's a mod called Heatsink. I'm not sure if it's been updated with recent trinkets, so you'll have to edit one of the files to add support for newer trinkets.
Heatsink isnt your best option you should now take up Needtoknow you can use to track internal CD's now along with its normal buff/debuff duration timers.
