Hmmm if you ask me Russia has a very nice system going( if your aim is to lower the population and stabilize drug use). The chance of contracting HIV is a deterrent to those young people who want to try drugs and have some fun, and the user who are paying drug dealers and therefore supporting the drug business are dying off.

Although I don't exactly like the latter, I think the HIV threat is a great deterrent.
