Kobe is leaning back, and Artest's left arm is above Kobe's right shoulder, and though it's difficult to tell, it appears his elbow may be near Kobe's head. At 0:23, Kobe swings his right arm back, generating quite a bit of force as his entire body turns. It appears to hit Artest in the throat. Artest immediately reaches for his throat at 0:25.
