I double checked and I believe you are mistaken. Letting the mines kill you rewards you with ending "C" on the title page.  Also Letting the submarine's torpedoes kill you rewards you with ending "D" on the title page. Both of which I show in the video. If I am misunderstanding, please elaborate further.

1. Look at a balloon.

2. Drown the guy on the jet ski.

3. Let the guys on the raft blow up all three of your eyes with mines.
