The maleria is not too bad I just sleep alot and avoid huge fire fights and pick them of with snipers and iIEDsnd the AK47 is my fav weopon its much better with an accuracy upgrade.

It's pretty much random... Although outside forces can affect it, such as multiple battles and lots of damage taken.

Often enough but u get used to the time in between, so you never run out but have to complete side missions in order to get more

actually the malaria you get in the game is unavoidable, its just one of those things that depending on how you play the game, effects your character.
