Alexx was exposed to gases that knocked her out for a while.

2:16 - 2:25 was an episode called "Bang Bang You're Debt. Its from the 6th season and 2:50 is from the fifth season. I don't know the episode name but I think its 5x07.

3:40 is from the sixth season. The episode is called Deep Freeze and Natalia's shoulder is knocked out of socket and Eric has to put it back for her.

But seriously, in season 2, there was a case that used a spear arrow, I believe and when Calleigh shot it, the kick was so strong when it hit her shoulder that it bruised her.
