As to the in game effect, well, it is a little unfair, but you can also consider it a match up difference, although it's too early to judge how it effects balance.

While I do agree the pain and damage is the same, the center of gravity differences between men and women would make a women fall backward. I could be wrong but that's how men and women fall when fainting or collapsing in a drunken stupor. The heavier bones in male shoulders are above and FORWARD of the center of gravity and the heavier bones in female hips are beneath and BEHIND the center of gravity.Still, I could be wrong, perhaps the nature of the low blow could change the falls direction.

You're comparing the damage done to the surface nerve endings to the damage to the testicles. You're aware of the kind of pain that occurs to guys.
