It's kind of a misnomer to have a Health Czar, as well as all the other czars, since anything such a job title would cover infringes on what falls withing the HHS director's purview. And the cabinet in general has less and less a hand in policy decisions as things are more and more handled by the political section of the Executive Office of the President. Don't get me started on the mess they've made of the Intelligence Community, or of Civil Defense, by going with the FEMA model.
