It's not fake, it's a genetic disease called harlequin ichthyosis which causes the skin to grow much too fast and too thick. The child is not deformed as such, the skin is very tight and this has pulled its mouth into a circle shape. Those aren't its eyes you can see; again, the tight skin has caused the eyelids to turn inside out. Sadly, apart from the terrible skin, this is a normal child, but is highly unlikely to have survived.
