A couple of honorable mentions for the list:

System Shock (the first one). Graphics were a bit more primitive than in the sequel, but man alive that game had some creepy-ass moments. You really started to think SHODAN was an actual AI the way she kept setting you up. Really well-written storyline, too.

Bioforge: Sort of Planescape: Torment meets System Shock. More creep-you-out scenes than you could shake a stick at. Playing the flute and the frozen scorpion cutscene were my two faves.

For me it'd always been the part in MGS2 where you're playing as Naked Raiden, the guards suddenly become INSANELY tough in their new cyber suits, and the Colonel starts behaving erratically with his codec messages. I also found the sound of the Rhesus Monkey roaring and rattling the cage in the research lab in Parasite Eve II a bit disturbing too. But maybe I haven't played enough games I guess.

The school in Silent Hill wasn't really a moment, it was a whole section of the game. It def was creepy, but I can def think of some much creepier moments in the Silent Hill series. I know a lot of people didn't like SH4: the room, but there a scene in the beginning right after the girl calls to you over the loudspeaker in the subway, and when u get to her, she's been attacked. Then in the cutscene, she tells you "I feel like...I'm dying!" Idk what it was about that, but it freaked me out.
