  • I'm doing my clinicals at this horrible hospital where every phlebotomist ignores the rules...I am not getting the proper training at all. Every last of them shake the tubes, when i try to do venipunctures the right way they push me out the way saying i'm doing it all wrong...pray i make it through clinicals

  • The so called phlebotomist chose the wrong vein and left the tube in while taking out the tourniquet first which can lead to a hematoma. Do they really think this is awesome? I am a CPT-1 instructor and this was one of worse draws. I will show my students this video on how not to draw. The other points above are well pointed out.

  • Okay, I'm sorry, but that was not "awesome". You did not wear gloves from the beginning, you shook the tube after the draw instead of inverting it, and you should NEVER leave the butterfly needle just sit there in the vein. Two fingers should always be on it to secure it.

    By the way, why didn't you use a syringe or a vacutainer? His veins are visible from a mile away. A blind person could've drawn him!

    Other than that, good job. Keep practicing and you'll get better :)

  • I guess he shaked the tube to mix the blood with the anticoagulant but that shouldnt need so much shaking n yeah y use butterfly needle for veins like that. n i'd probably prick along the route of the vein and not at an angle to avoid double puncture.

  • THANK YOU! As for you MR. FROZEN EMT(Wanna Be Know It All) We are talking about venipunctures here(Withdrawing Blood) Who would want to leave a tourniquet on for up to 4-5 hours to draw out blood? No hard feelings im just saying.

  • Wow he has really good veins and actually when it comes to butterfly needles u can let go of it u can just put tape if you'd like and also your not supposed to shake the tube like that either. also when you get your flash and start drawing your supposed to let the tournequet loose because if u dont it will cause a hematoma. DONT leave the tournequet on for no longer than 2 minutes!!

  • I dont know where you trained, but absolutely everything you said is wrong. You might want to go back to school. As for leaving the tournequet on for more than two minutes, no worries. If you were an EMT, you'd know you can leave a tournequet on muscle for 4-5 hours without causing damage or necrosis (thats dead tissue since Im assuming you didnt take medical terminology either).

  • Umm, I'm in nursing school, and we're taught never to leave a tourniquet on for longer than 2 minutes else it causes hemolysis (blood cell breakage) causing K+ to leak, and might result in fluctuation of your lab values. I've never had any EMT training, but considering that field, I'm thinking... maybe tourniquets placed on for that long is perhaps used more for shock management and bleeding control. But you certainly wouldn't leave it on that long for a regular blood draw.

  • you can NOT leave a tournequet on for more then a minute when doing venipuncture because it will cause hemoconcentration and screw with the lab results...has nothing to do with causing necrosis

  • @enligh10dwon it can cause hemoconcentration, but rarely. the main reason for the 1 min rule is to prevent hemolysis

  • I know what you mean. Im starting med asst training next summer. The big thing is just not to be afraid. Are you going to hurt some people? Yes. Even the best do. And if you think your veins are going to roll, you can help prevent that by pulling the skin tight on either side of your selected vein and it helps hold it in place. But still, some veins insist on rolling, especially in old people where the veins are "harder." Dont worry. Soon you'll be sticking like a pro! Good Luck.

  • a neighbour of mine died during a blood drawing!The assistent did not find his vein and he decided(very strange!!!)to cut him open and then at the sight of the vein to introduce the needle but by mistake he cut all his veins!!!This assistent forgot one thing...my neighbour was hemofiliac

    we only use bufferflies for little kids and when drawing from small veins.

  • Thank you. I'm glad somebody noticed that. And why would they use a butterfly for a vein that size? Shaking the tube decreases the acuracy of the results. They could had turned it upside down and than right side up a few times. Other than that, it was a good draw. Good asepsis. Good technique.

  • hell the last time I had a blood test done on me, the doc almost ended up swallowing a needle and crapping out my boot, damn bastard had no idea how to do it right, there sure as hell isnt going to be a vein in my damn shoulder. the result got me kicked out of the damn doctor's office, needless to say i needed new shoes after actually putting my size 12 boot up the doctor's ass. I am actually surprised I didn't get arrested for that, or for force feeding the doctor the same needle.

Way to go:) They probably didn't press charges on you, because you probably coulda pressed charges on him for assult and battery for what he did to your arm. If he'd a damaged a nerve, you woulda been able to sue the sorry bastard.

  • She really didn't need to use a butterfly for that huge vein! I work at a clinic as a phlebotomist and we only use butterflies on small children, hands, and elderly with rolling or hard to find veins. Plus, she did the prep w/o gloves! Bah!

  • @wallis0105 Well... It's mostly advised not to leave the tourniquet one. What happens when you have a 13 tube draw and the person has frail veins? You going to take the tourniquet off and lose your draw or stick a sick person twice?

  • the butterfly needle is a winged needle attatched to a tube, whereas a normal needle is just a needle and hub with no line.

    the butterfly is usually used for patients with small and hard to locate veins as the needle is normally smaller. It's also used in patients that need many tubes drawn because it allows the person taking the blood to be able to move more freely because it has a line attatched to it.

  • A butterfly infusion set will put less vacuum on the veins because of the tubing. If a person has veins that are more likely to collapse, then a butterfly should be used. A standard butterfly needle is a 23 G. It is very small. A standard multi use needle is going to be a 20-21 G which is bigger.

  • A person who has not drawn blood for years at a time should NEVER put the cotton ball over the needle when pulling the needle out, that is how you get a dirty stick.

  • well... pretty good.but not perfect.You should wipe after you put a tournequet on because you could accidently touch the area and spread bacteria.Also you should not rub the area with the alchol prep because that is also picking up bacteria and spreading it around like butter.Swipe in a 12 o'clock motion start at the top and swipe 0ne circle back to the original position.also you shouldent leave butterfly just hanging there always hold on to it and never take your eyes off of it,even to push tb.

  • Great video! Awsome arm!! When you draw blood, you need to prepare the items you will be using just like in this video. You DO NOT need to put gloves on at the very start. First you need to put the tournaquet on and palpate for veins, when you are ready to poke then you can put your gloves on. I've seen alot of people ask where's your gloves??

  • I despise using butterflies. I always have to tape mine- ligltly near the needle and on the tube part so that nothing moves while inserting tubes.

    Ole girl shoulda used gloves from jump start, cleansed the entire area (i use swipes across entire AC incase somkethign doesnt go right).

    IDK butterflies make me nervous. and ole boy has awesome veins!!

  • I have small vains so they always use the butterfly on me. They work better and to me at least to hurt that bad. I have been stuck with a straight needle quit a few time's. Although they do ok with me I prefer the butterfly.

  • It was an ok draw but we were taught to ALWAYS cleanse the site in a concentric motion & to NEVER wipe away the access alcohol; let it air dry w/o fanning, blowing, or wiping the alcohol. It was an adequate draw, but it needs some improvement.

  • why use a small and expensive butterfly for such large veins? what's wrong with a plain ol' vacutainer needle? Or just for fun, like a 14g angiocath?

  • QUESTION - Why the hell are they using a 23G butterfly for at least 4 (including the one they used) perfectly good veins that they could straight needle with a freaking 21G without blowing if they were skilled? In other words - this is not awsome, this is bullshit.

  • OK. Out of all the blood draw videos I've seen, this is the most accurate.  A few changes needed, but she did awesome. She shouldn't have used the butterfly with your amazing veins. They are not good for samples, expensive, and dangerous for the phlebotomist. (responsible for 32%+ needlesticks) With your veins, the basilic vein didn't have to be used. It is the "danger vein" when it comes to nerve injuries. Lastly, cotton is frowned upon. Otherwise, KUDOS to that phlebotomist!

  • I agree with Kansascaptain about 99%. I don't give kudos for the simple reason that the friggin veins on this patient were amazing, and the phlebo. chose the friggin basilic. Dangerous and unnecessary. And cotton? Have we read up on the current NCCLS guidelines? I think not.

  • I am a phlebotomist, I draw blood all day, every day.

    A few observations...

    1 - The tech should have FIRST put on gloves, then applied the tourniquet, then searched for a vein, then wiped the site with alcohol. Gloves MUST be put on before ANY patient contact, even if its your own mother.

    2 - The use of butterflies is way out of hand. They should only be used for babies and blood cultures. THAT'S IT.

  • But how is this differnt than when a nurse noramlly takes out blood? I just came from the drs and they took out 4 vials of blood but they didn't do it like this. How is this different than when the nurse usually takes out blood? Thanks for the answers.

  • Slight sting to almost no pain at all. Butterfly needles are very thin, and the median vein which is shown being drawn from here has a lot less nerves than other areas. The guys veins in this video are very nice veins, and a phlebotomist's cakewalk.

  • The SST should also be inverted to mix the sample with the clot activator. As long as it is a gentle inversion, the sample will not hemolyze.
