I see three things he does wrong, takes side step which messes up his balance, he twists his pull-up leg towards second base and lastly he lands wrong, too much to the right probably a result of aforementioned twist so he's throwing a bit across his body. To correct it, he should step straight back and probably everything else will fall into place. His pitching coach should try that once he is back from rehabilitation.

@stirling513 Pitching overhand is a lot better for your arm in the long term. You are supposed to ice your arm after every time you pitch, rather it hurts or not. Ice it on your elbow for atleast 15 min, than your shoulders.
