To answer your question, Akito is a girl. This is explained in the manga but not in the anime, which only goes until about volume 6 or so in the manga's story line. The Sohma family forced Akito to pretend to be a boy so that she could be a strong head of the family. There's more details to it, like how her mother was crazy, but I don't remember all of them.
