In the books he tricks her by pretending to be a priest who is hit on the street so she invites him into her house, and though this is unlikely in a modern remake, he is wearing a black shirt so this cannot be ruled out.

The last scene is obviously HOUND, obviously in Baker Street, so early on. Apparently it's after the point where Sherlock is uninterested when the case is introduced to him, so around the point that Dr Mortimer has explained Charles Baskerville's demise.

As the scene of John punching Sherlock is part of the Scandal in Belgravia trailer, it's safe to assume that it has something to do with Irene Adler. She's been portrayed as a dominatrix and offers to hit Sherlock, so he could be hoping to pretend Adler was the cause, or else seem as if he is a masochist for the purpose of tricking Adler.

The person who saves them is either a stranger who is heretofore disconnected witht eh plot, or a lead in a subplot where he has been watching Sherlock's movements carefully. Either way it is unlikely that he will be there purely to save their lives and then leave. Television doesn't leave simple heroes alone.

The bus is a 74, which goes from Putney to Baker Street. Looking at the street, I'd say they aren't far from Regents park, which could explain the bus simply due to the proximity to the main setting of the TV series. However, it could point to the bus carrying soemthing in the direction of Baker Street to target Sherlock and John personally, be it a person, a bomb, or some object they are chasing like the Maltese Falcon.

Sherlock pulls John into the road, but John grabs his hand to pull him off the other side. the fact John doesn't initially object implies that the bus they are standing in front of holds some significance, that they probably need to stop it. Why would they need to stop a bus? If it's to do with Moriarty there's a strong possibility of some bus-bombing action, as the London 7/7 bombings are fresh in the minds of many Londoners [myself included] and Moriarty is a bomber in this seres.

@fleuve0styx I think they're both handcuffed AND holding hands. There's a rumor that they end up handcuffed and decide to hold hands because it makes it easier to hide the cuffs under their sleeves, so that no one sees them.
