Look carefully, he is braking with his right hand. It looks like his left hand is a throttle control. He must have some kind of physical issue that keeps him from using his feet so he is using hand controls.
It seems to me that he has a second lever, just to downshift. As it is closer to the steering wheel, it is faster to move your hand from the wheel to the lever and vice-versa.
It's just like some cars in which have the steering wheel paddles (we call them "borboleta" - butterfly, in portuguese) and a stick. You have only one transmission and two ways to change gears. Hope I have helped...

What is the lower handle he keeps getting with his right hand? It would seem he would want them both on the wheel more.
