No, it's a computer program developed by UnFun Games. And it is real. This song was made using a alternative soundfont created by a user right here on YouTube. And you're correct. It isn't Mario Paint, It's Mario Paint Composer, which is better than the original game for the SNES, because this version allows Sharps, Flats, Higher Tempos, File Saving/Loading and they even expanded the measure count. Maybe you should try it out.
Well, there are a few things I can show to try to prove that theory wrong. First, it is slightly faster than the original (due to tempo gaps in MPC). If you pay really close attention at the end of each file, the percussion will do a weird lag thing. The very first note of the melody line should have harmonics so that you can hear multiple octaves (I tried to emulate this using the pig, but you'll notice it still sounds slightly different).
Not fake, as I can tell from a couple of very subtle lags. But this really sounds awesome; veery identical to the original sound bite. Excellent. 5 stars 4 u! :)
I am sure that many people never played this game and have no clue how dead on this is to the original. I thought I was going to post the first MPC Faxanadu music, but I was wrong :P. I want to go play this game now! Great job.

This isn't real.
The yoshi's don't make any sound. The stars don't make any sounds, and the cats certainly don't make any sounds.
Even the bottom bit isn't mario paint. Must best some hacked computer mod of mario paint or something.
