Luckly the worse I ever done was leave out a G01 by accident with the last movement command being a G00, needless to say, i never seen a 3/4 inch carbide end-mill explode like that.
Glad to know that no one was hurt. Although not in your case, we have seen a holder fly out of the spindle on a brand new machine that was only in it's first spindle warm up. The holder came through the metal shrouding on the VMC at the base. Needless to say it scared the crap out of everyone. It turns out that the cause of it was a poorly heat treated retention knob that was 'on sale' It was our holder but NOT our pull stud (which we where glad to find out.) some things u can't cut corners on
How hard is it for people with an iPhone to turn the phone sideways when they take video? I can't stand this video format. It doesn't fit on any screens besides the iPhone properly.
Well, why don't you communicate the reason for this!? Was it operator's error or program error(if so, why it was not proofed before run? and why it was ran full speed?) or was it machine's mechanical/electrical malfunction?
After 30+ years on CNCs I can tell you, all this is preventable with proper work methodology.
@sonofsun4 As I was reading your comment and got to "30+ years on CNC" my thought process went, "Wait a minute, 30 years on CNC, it's not been in mainstream use that long...oh my god it's 2011 and 30 years ago was only 1981, god I'm old" ahahaha This growing old stuff SUCKS. erklaerbaer01 : the programmm went to face the top of a peice then return home in rapid, the table was suppose to turn for the tool to come in and face the other side, the table didnt turn, the tool came in in rapid right on top of the piece.

How did you acomplish that? broken pull stud? or did the hydraulics screw up? or the tool holding collet?
