He just crammed a clone system inside an NES cartridge. Listen to the audio on the game. It does not faithfully reproduce the sound. He used a clone system.
Make a second one and plug it into this cardridge. Then make a third one and plug it into your second nes in a cardridge. Eventually you will plug in the first nes in the 4th nes. Infinite gaming loop! Now youre playing with power!
No most likely he gutted a NOAC (Nintendo on a Chip) NES Clone, which would of had it's own cartridge slot. A NES cart is too small to fit even the NES-101 (the smaller top loader NES) inside. NOAC's can be really small, for example the FC Mobile II

at 1:54 i can see you've removed the original pcb of that cartride to do this mod, i've seen that some games (i'm not a big nes player) has ridiculously small pcbs compared to the cartridge's size. You know what could be fun? leave inside one of these, so you can plug your nes-on-a-cartridge on an other nes or build an extension cable so you can even plug it to itself and actually play! Now THAT would me mindblowing :D
