The is the music of the final duel and it is the epitome of the so called Good vs. Evil. Tales and epic stories have been written of this and of the fairness and frustration that is so perfectly portrayed by Lee Van Cleef each time he glances down at his revolver. Eastwood's character is considered somewhat supernatural and here, he clearly emerges as the Hand of Fate holding the musical watch. Spooky and chilling are the words to describe the duel, but that music is beyond description here.
@Ghoststar2009 Harmonica in Once Upon a Time in the West is often considered supernatural. The shot of him getting up after being shot after being shot in the opening scene was originally cut, then added back in by other people. It is a popular theory that he represents all of Franks victims, which is why he lists their names every time Frank asks him who he is.
It is interesting to consider that Eastwood had a similar role here. Remember, Leone originally wanted Bronson to star in these movies

You are correct - the organ is not in the final Indio- Mortimer-Eastwood duel, a trumpet is used, signifying the final duel is indeed, at hand.
