From the graphic novel Cabtain America: Reborn, Red Skull wanted to takeover the body of Cap so he got Sharon Carter to trap Cap's consciousness in a time trap which looked to the world as if Cap got shot. Skull with the help of Zola, Doom, and Osbourne use one of Doom's devices to put Skull's consciousness into the recovered and healed up body of Cap. Cap's consciousness eventually overpowers Skull, and he thus now has a healed up body again. He allows Bucky to be Captain America anyways.
