It's a bit easy to tell, especially at the "And remember when I weighed one thirty-five", "This one's of me, throwing up for you, and I'm paler still, but that's the way you wanted it" as if he's talking to Mia. Often times, people refer to an ED as an actual person, because that's what they see them as; Someone making them feel horrible, making them change, making them feel not good enough. ^^;
