That's the Epitaph Blade. You'll need an...Ancient Flatstone, I believe. Whichever Ancient stone nets you a Greatsword. If you're lucky, it'll turn into a Weathered Greatsword when you take it to the weapon/armor guy. From there you can turn it into either an Epitaph Blade or a Teostra Blade.

Now then, is the guy at 4:40 wearing Teostra armor?

you get it from polishing rusted stones. The rusted stones are from just about any G-class mining spot (They are rare). Just keep getting them and going to the weapon shop to turn them into weapons. (They are the ????? weapon in the weapon shop) Eventually you should get a worn blade(or something like that) which upgrades into the weapon epitaph blade which If i'm not mistaken is the one shown
