You get it with an aimbot, and on tf2, an aimbot most likely has a crithack. Considering the server is empty, and your not doing this in a full server, it could be a console command.

He coded this a long time ago. Some hacks have a feature (and most should) that take the visual effect of nospread and recoil away, so it looks normal. This hack had that feature implemented in the nospread or as a seperate feature. I used to use this hack a long time ago, the coder isn't working on it anymore, it's long gone.
