Fascinating stuff and lovely anecdotes. As I'm teaching myself (and can only blame myself) I find the Japanese language to be a delightfully layered onion: each layer yields another and it sometimes brings me to the brink of tears. :o)
I've noticed that rabbit-in-the-headlights look even here in London with Japanese tourists - and it does seem to be a common theme. It's as if they've been led to believe that mistakes are a source of shame rather than entirely natural. It's a damn shame.
And oh yeah, I know what you mean about people saying something to you, and you need to have them repeat. It breaks my heart, because I want to know what they want to say. Even worse when they need to repeat multiple times. :(

Great personal account! Thankfully I've never had any similar experiences at a マクドナルド or other eatery.
