There are some doctors in Australia but I do not have the list. Please let them know our distributor in Australia is Ellex in Adelaide. Their phone number is 61-8-8104-5293. Ellex will be able to direct them to a doctor near them.
”You may have partial blockage creating a sludging or stagnation of meibum without complete obstruction. Probing will help mobilize meibum secretion to allow enhanced flow of existing secretions. Also would recommend use of intraductal steroid microtube injection to control inflammation and promote increased synthesis and secretion.”
@rodrii14 a doctor would also suggest hot compresses pressed along the eye lid to help disove the olis which are clogging up your system dont let a guy mess with your eys over a simple solution such as placing your face over a hot water source for a few sewconds. such as a face humidifier
@etrax2000 I absolutely agree, why surgery. I started taking Maxitears (got it on Amazon) and after 2 months see amazing improvement already. No surgery for me
@latifi1 I also have used the gel GENTEAL and the liquid poured into the lens istead of the baush and Lomb which I believe the preservative in it may cause the eyes to dry and become painful.. this treatment has made wearing the lens more comfortable for rmore hours.. it's all a matter of just the loss of moisture in the eyes, don't let some pill poppin doctor run the risk of loosing your eyesight. I realize there are times when it seems that you want to pluck out tour eyes beacuse they hurt.
Your brief history suggests a variety of dry eye related disorders. Would recommend a good evaluation and targeted therapy which may include meibomian gland probing. My office is in Tampa, Florida.

I have been diagnosed with Blepharitis in which my Opthamologist believes stems from my Rosacea. I get a white sticky gooey muck in the corners of my eyes ion a daily basis in which he belives is a buld up of dirt and grime to which is irratating my eyes. What is this stuff called? Would this treatment on this video help me?? My eyes are always bloodshot, tired looking, gritty and terribly embarrassing as it appears I have been on drugs or drunk heaps of alcohol.
I started having very dry eyes a few years a go which at that point I stopped using contact lenses. I have been doing a lot of research for 3 years and today I looked under my upper eyelid and I did see a red line with uneven bumps on it which is a MGD symptom. I always over wore my contacts and sometimes they were very dirty which encouraged more oil to be released. If my MGD was caused by contacts, will probing fix it permanently or can it come back again?
By removing obstructions and establishing an open ductal system through intraductal probing, the lipid secretions are able to flow out through the orifice to reach the tear film. This helps create a healthy functioning gland and target tissue relationship. In the setting where the gland outflow is obstructed then lipid secretions stagnate, creating inflammation, breakdown of tissues and ultimately scarring with gland atrophy.
So would you say that it is necessary to treat the underlying disease (to the extent possible) and the dietary issues before opting for this treatment?
The final common pathway in these diseases is increased keratinization, thickened meibum and inflammation with fibrovascular changes. These changes then begin a vicious cycle of obstruction followed by increased inflammation. Probing can immediately break this cycle, open up obstructions and establish a patent ductal system for healthy gland function. Treating co-morbid disease can then help reduce risk of re-obstruction.
As stated previously, I believe the initial obstruction creates a vicious cycle of inflammation which exacerbates the obstruction leading to even more inflammation and fibrovascular changes. Removing the co-morbid disease does not address this. If it did then obstructive MGD would not be the bane of generations of dry eye patients.
I understood what you were saying -- that treating
the co-morbid disease does not eliminate the
exacerbated obstruction/ inflammation. However, I still believe that treating the co-morbid disease would alleviate the initial obstruction, thereby preventing its exacerbation, as it is the co-morbid disease that is causing that obstruction.. If this is incorrect, please let me know. Thank you.
For example, if patients have allergy, aqueous deficiency or anterior blepharitis, I would recommend treating these diseases to see if associated MGD responds. However, many times the associated or co-morbid diseases are successfully treated and yet obstructive MGD persists.
We use topical anesthesia to minimize discomfort. Length of procedure depends on severity of case and number of glands/lids involved. This procedure is usually done in the office exam room. Reviewing data of the original 25 patients with now about 18 months follow up show that the majority do not need retreatment (about 70%). The approx 30% who received retreatment did so on average at 8.5 months post procedure.

Hello,
I have reduced production of oil from the meibomian glands. I think they are not blocked, they just produce oil in little quantity.Could this help?

Thank you for your work and video.
I wonder, however, if this treatment addresses the intraductal dysfunction? Do you know what causes that?
Thank you.

Thank you Dr. Maskin.
However, I was wondering if you knew what causes the obstruction in the first place?
Super5546 1 year ago
Many co-morbid local and systemic diseases could cause obstruction whether functional or anatomic. Nutritional and dietary issues likewise.

Thank you again.
I must, however, wonder whether treating
the co-morbid disease wouldn't suffice to
break the cycle and open the obstructions, rather
than using an invasive procedure to establish
a ductal system? It seems to me that this is
treating the symptom, not the cause?

The video says improvement lasted for 4-6 months. Are you sure about those 8.5 months ?
Do you know if this is available in Europe/Spain ?

Yes, now that we have longer follow up we can get a better picture of the duration of effect and rate of retreatment.
